Massive asthmatic atelectasis.

چکیده
Atelectasis as a complication of bronchial asthma occurs more commonly than is diagnosed or suspected. It is usually of the lobular type and patchy in distribution. This type was first described by Huber and Koesler in 1920.’ Many authors have suggested that status asthmaticus and even death occurring during an attack of asthma are related to lobular atelectasis.2’ Massive atelectasis, on the other hand, is much less common than the lobular type. Since Clarke4 first described massive asthmatic atelectasis in 1930, there have been six reports published5 6, 7. 5, 9 10 describing a total of 15 cases. The purpose of this communication is to present two more cases and to review the clinical aspects of this uncommon complication of a common disease.

chospasm and edema of the bronchial wall. The problem of plugs is compounded by the invariably present tachypnea, resulting in respiratory loss of fluids. Dehydration produces tenacious, inspissated plugs, which are difficult to remove via the cough mechanism. Messer et al,1° in a large autopsy series, reported a 97 percent incidence of abnormal bronchial contents (mucous, purulent exudate, or ...
متن کاملUnilateral pulmonary collapse in asthmatics.
Five asthmatic patients developed collapse of one lung. Three of the patients were children and three of the five had repeated episodes of atelectasis. Episodes of atelectasis were usually associated with localised chest pain, which was not pleuritic in character, and with breathlessness, but without wheezing. The were not related to clinically apparent respiratory infections or to deterioratio...
متن کاملRelating maximum airway dilation and subsequent reconstriction to reactivity in human lungs.
Measures of airway resistance (Raw) during deep inspiration (DI) suggest that asthmatic subjects possess stiffer, more reactive airway smooth muscle. There is evidence that one can enhance airway reactivity in healthy lungs by prohibiting DI for an extended period. The present study had two goals. First, we determined whether the maximum dilation capacity of asthmatic subjects depended on the r...
متن کاملSmooth muscle hyperplasia in an asthmatic patient: do we know it all?
Bronchial smooth muscle hyperplasia is a well-known structural change in asthma. The degree of hyperplasia is related to asthma severity. We report a case of extreme smooth muscle hyperplasia in an asthmatic patient. A 54-year-old female with a diagnosis of analgesic-induced asthma was admitted to our center for nasal polyposis surgery. During her preoperative evaluation, atelectasis of the rig...
متن کاملRespiratory support with nasal high-flow therapy helps to prevent recurrence of postoperative atelectasis: a case report
Postoperative atelectasis should be avoided in surgical patients with impaired pulmonary function. Nasal high-flow (NHF) therapy delivered by the Optiflow™ system (Fisher & Paykel Healthcare Ltd., Auckland, New Zealand) is a new, simple device that supplies heated and humidified oxygen gas at >30 L/min via a large-bore nasal cannula. We herein describe a case in which respiratory support with N...
